Management Considerations for PTA
Acute - Protection Phase
- instruct in use of ambulation aid
- instruct in adapting seating surface to decrease hip flexion ROM and WB forces during transitions
- AAROM/AROM in all planes within pain-free range
- consider aquatics or conditioning exercises with minimal joint impact
- observe post-operative hip precautions when indicated
- modalities PRN for soft tissue repair and healing
Subacute - Controlled Motion
- Strengthening (typically gluteus medius, maximus, hip rotators) is progressive with WB/closed chain stabilization as tolerated
- Stretching involved structures which have restricted length
- Soft tissue mobilization to enhance scar mobility
- Postural retraining
- Low impact aerobic exercise
- Patient education in maintaining balance between strength and flexibility
- Patient education in post-operative precautions and signs and symptoms of surgical failure
Return to Function
- Patient-specific. Focusing on return to work or recreation activities at or near previous level of function
